Introduction

ActualMeds Corp. is committed to protecting your privacy and the security of your data. This security and privacy statement applies to the data collected by the KnowMyMedsTM system and any account or ID you establish (called the “Service” below). It does not apply to data collected through other online or offline sites, products, or services.

The Service is a personal health solution that lets you collect, edit, review, store, and share health and medication information online. With the Service, you can control your own information. You can also share your health information with family, friends, and health care professionals, and have access to online health information and management tools.

You can store health information for other people (such as a family member) by setting up a shared “Caregiver” login. You should store and access the information for each person for whom you may be a Caregiver in a separate Service account.

Collection of your personal information

The Service asks you to enter a login name identifier and password to sign in. The Service also asks you to provide other critical identifying information in order to build an account; this includes personal information such as name, date of birth, e-mail address, address.

We will use your e-mail address  to notify you of successful account setup, to send you Service notifications and updates.

An account allows you to manage one or more SmartListTM, such as the ones you create for yourself and your family members. You choose what information to put in your records. Examples of the types of information you can store in a record include:

  • Symptoms and health conditions
  • Medications
  •  Information about what helps you manage your health

By default, you are the manager of any records you create. You may designate additional people to be co-managers of this information by designating them as a Caregiver. Some of the information stored in the records you manage may be highly sensitive, so you need to consider carefully with whom you choose to share the information.

Sharing your personal health information

A key value of the Service is the ability to share your SmartListTM information with people and services who can help you manage your health. For example, you can share health information from records you control in order to assist a family member in managing their health, or to consult with your health care provider as a part of your care.

How we use your personal information

We use personal information collected through the Service, including health information, to provide the Service, and as described in this privacy statement. We do not use or disclose your information except as described in this privacy statement.

Personal information you provide may be used for the following:

  • to provide you with important information about the Service, including critical updates and notifications
  • to send you the KnowMyMedsTM e-mail newsletter, if you opt-in
  • to generate the customized, personalized SmartListTM based on your responses to questions.

Personal information collected on the Service is stored and processed in the United States of America.

How we use aggregate information and statistics

ActualMeds Corp. may use aggregated information from the Service to improve the quality of the Service and for marketing of the Service (for example, to tell potential partners how many Service users live in a particular US state ). This aggregated information is not associated with any individual account. ActualMeds Corp. does not use your individual account or record information from the Service for marketing without first asking for and receiving your opt-in consent.

Account access and controls

You choose whether to create an account with the Service. The required account information consists of a small amount of information such as your name, e-mail address, region,. We may request other optional information, but we clearly indicate that such information is optional. You can review and update your account information. You can modify, add, or delete any optional account information by signing into your KnowMyMedsTM account and editing your account profile.

The Service allows an account to have two possible users – the  primary user, and a possible caregiver. This feature enables family members or professional caregivers such as visiting nurses to create and manage records on behalf of someone.  You may also create a second login to be shared with a family member or caregiver yourself. Because inappropriate granting of access could allow a grantee to violate your privacy or even revoke your access to your own records, we urge you to consider all the consequences carefully before you grant access to your records.

E-mail controls

To keep you informed of updates and improvements, the Service will send periodic email newsletters. If you do not want to receive the newsletter, you can uncheck the box that indicates a willingness to receive these updates when you sign up for the Service, or check the box in your account profile page at any time. If you later decide that you want to receive the newsletter, you can request it by checking the box on the account profile page.

Security of your personal information

ActualMeds Corp. is committed to protecting the security of your personal information. We use a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, and disclosure. We adhere to security and privacy standards and best practices, including HIPAA.  For example, we store the personal information you provide on computer servers with limited access that are located in controlled facilities. Additionally, the service sends all communications, except e-mail, using encryption (that is, HTTPS).  Any stored data is encrypted. No personnel related to the Service will have access to your sensitive health information, unless you explicitly grant it in resolving a technical or functional question.

Changes to this privacy statement

We may occasionally update this privacy statement. When we do, we will also revise the “last updated” date at the top of the privacy statement. For material changes to this privacy statement, we will notify you either by placing a prominent notice on the home page of the KnowMyMedsTM site or by direct notification. We encourage you to review this privacy statement periodically to stay informed about how we are helping to protect personal information we collect. Your continued use of the service constitutes your agreement to this privacy statement and any updates.
